Entries may only be made by current RCM members

Entries can be from individuals or teams working in any setting and at any level

You should describe your project in no more than 500 words

Cost effectiveness is a key component of a project's success and sustainability so you should ensure this forms part of your submission

All submissions that accompany the completed entry form must be completely anonymous with no reference to either yourself, colleagues' names or place of work

Your project must only be entered for one category

Any entries that have previously won awards will not be considered

All entries must be signed by your head of midwifery or employer.

Only two individuals may be named on the award application

The individuals named on the entry must be available to attend the judging day in the event of shortlisting.
